摘要:

目的 探讨基于全健康(One Health)理念的血吸虫病智慧化监测预警体系在消除地区的应用架构与实施成效,为同类地区巩固血吸虫病消除成果提供参考。方法 2024年浙江省常山县建立由政府主导、多部门协作的血吸虫病监测预警组织架构,上线血吸虫病智慧化监测预警系统,对“人-动物-环境”开展一体化监测预警,涵盖人群主动监测、医院就诊监测、宿主动物监测及螺情监测等4个模块。收集分析该体系2024—2025年的多源监测数据,并与历史基线数据进行对比。收集传播途径、传染源、防治能力、人群脆弱性、自然因素等5大类的22个指标,计算10个行政村的风险顺位,评估其在风险识别、预警响应、控制措施等方面的效果。结果 成功构建了“政府主导、多部门协同、数据共享、智能研判”的常山县血吸虫病智慧化多点触发监测预警体系。实现了对人、家畜、宿主动物、钉螺、环境和输入性风险因素的全天候、自动化监测与数据汇聚。系统累计自动采集医防协同数据15.4万条,累计筛查重点人群25 303人次,预警响应时间较传统模式平均缩短3 d,从数据触发到预警信息推送至管理人员的平均时长缩短至8 h以内,重点环境精准查灭螺效率提升约40%,家畜监测覆盖率达到96.7%,成功预警并处置输入性风险事件11起。10个行政村的血吸虫病传播风险顺位中4个高于0.2,5个介于0.1~0.2,1个低于0.1。结论 基于全健康理念的智慧化监测预警体系能够有效整合碎片化信息,显著提升消除地区对血吸虫病输入与再发风险的早期感知、快速研判和精准干预能力,是迈向并维持可持续消除的必然选择和有力工具。

Abstract:

Objective To explore the application architecture and effectiveness of implementation of an intelligent surveillance and early warning system for schistosomiasis based on the One Health concept in elimination areas, so as to provide insights into consolidation of schistosomiasis elimination achievements in similar areas. Methods The organizational structure of government-led and multi-sectoral collaborative surveillance and early warning for schistosomiasis was established in Changshan County, Zhejiang Province in 2024, and an intelligent surveillance and early warning system for schistosomiasis was run for integrated surveillance and early warning of human-animal-environment, which was consisted of four modules, including active surveillance of humans, surveillance of hospital visits, surveillance of host animals, and snail surveillance. Multi-source surveillance data were collected from the system from 2024 to 2025, and compared with historical baseline data. A total of indicators from 5 categories were collected, including transmission route, source of infection, control capability, population vulnerability, and natural factors, and the risk ranking of 10 administrative villages was calculated to evaluate the effectiveness in risk identification, early warning and response, and control measures. Results An intelligent multi-point triggered, government-led, multisector-coordinated, data-shared and intelligent-analyzed surveillance and early warning system for schistosomiasis was successfully established in Changshan County, which achieved all-weather, automated surveillance and data aggregation of human, livestock, host animals, snails, environments, and factors of imported risk. A total of 154 000 pieces of data pertaining to integration of medical services and prevention were automatically collected by the system, and a total of 25 303 person-time high-risk populations were screened. The time of early warning and response reduced by a mean period of 3 days compared to traditional modes, and the mean duration from data triggered to early warning information sent to administrators was shortened within 8 h. The efficiency of precise snail control increased by approximately 40% in key settings, and the coverage rate of livestock surveillance was 96.7%. A total of 11 imported risk events received successful early warning and response. Of 10 administrative villages, 4 had schistosomiasis transmission risk ranking of higher than 0.2, 5 between 0.1 and 0.2, and 1 below 0.1. Conclusion The intelligent surveillance and early warning system for schistosomiasis based on the One Health concept may effective integrate fragmented information and significantly enhance the capabilities of early detection, rapid assessment, and precise interventions for imported and re-emerging risks of schistosomiasis in elimination areas, which is an indispensable choice and a powerful tool for achieving and sustaining elimination.
