日本血吸虫成虫表膜抗原的诊断效果及疗效考核价值

EVALUATION OF SCHISTOSOMAJAPONICUM TEGUMENTAL MEMBRANE ANTIGEN (SjMAg) IN IMMUNODIAGNOSIS AND EFFICACY ASSESSMENT

摘要:   [目的 ]探讨日本血吸虫成虫表膜抗原 (Sj MAg)检测日本血吸虫病患者的特异性和敏感性及考核疗效价值。 [方法 ]采用 Sj MAg- EL ISA检测治疗前、后不同时期血吸虫病患者血清中特异性 Ig G、 Ig G4及健康人、并殖吸虫、华支睾吸虫及囊尾蚴病患者血清中的交叉抗体。 [结果 ]Sj MAg的敏感性和特异性与可溶性虫卵抗原(Sj SEA)比较 ,无显著性差异 (P>0 .0 5 ) ;用 Sj MAg- EL ISA检测治疗后 12个月患者血清中 Ig G的阴转率为80 .0 % ,显著高于 SEA- EL ISA的 43.3% ,而检测 Ig G4的阴转率为 93.3% ,显著高于 Sj SEA- EL ISA的 6 0 % ,检测治疗后 2 4个月患者血清中 Ig G和 Ig G4的阴转率分别达 92 .9%和 97.6 %。 [结论 ]Sj MAg- EL ISA检测日本血吸虫病具有与 Sj SEA- EL ISA相同的诊断价值和较高的考核疗效价值。

Abstract:  Objective To evaluate the specificity and sensitivity of SjMAg in detecting specific antibodies in sera of patients with schistosomiasis japonica and in assessing therapeutic efficacy. [Methods]\ SjMAg-ELISA was used to determine the specific IgG and IgG\-4 in sera of patients with schistosomiasis japonica at different time points after chemotherapy. [Results]\ SjMAg-ELISA was as sensitive and specific as SEA-ELISA. The negative conversion rate of IgG detected by SjMAg-ELISA 12 months after treatment was 80.0%, being significantly higher than 43.3% by SEA-ELISA. The negative conversion rate of IgG\-4 detected by SjMAg-ELISA 12 months after treatment was 93.3%, being significantly higher than 60.0% by SEA-ELISA.The negative conversion rates of IgG and IgG\-4 2 yr after treatment were 92.9% and 97.6% respectively. [Conclusion]\ SjMAg-ELISA is comparable to SEA-ELISA in diagnosing schistosomiasis japonica and is more effective in assessing therapeutic efficacy.\;
