酶联环卵沉淀试验诊断日本血吸虫病的研究

ENZYME-LINKED CIRCUMOVAL PRECIPITATION TECHNIC(ELCOPT) FOR THE DIAGNOSIS OF SCHISTOSOMIASIS JAPONICA

摘要: 采用热处理超声干卵抗原环卵沉淀试验结合酶联技术的酶联环卵沉淀试验(ELCOPT),与常规环卵沉淀试验(COPT)、酶联免疫吸附试验(ELISA)和间接血凝试验(IHA)进行了比较。在粪检阳性患者327例中,ELCOPT阳性率为96.0%(314/327),COPT阳性率为94.2%(308/327),P0.05,无显著意义。平均环沉率前法为27.9%,后法为15.7%,P0.01,有极显著意义。ELISA阳性率为89.6%(293/327),P0.01,有极显著意义。随机抽双港大队106例,IHA阳性率为82.1%(87/106),与ELCOPT阳性率92.5%(98/106)比较,P0.05,有显著意义。用ELCOPT检测43例正常人,结果全部阴性。本文采用板井法ELCOPT,可提前24小时出报告,底物以0.4%联苯胺为宜。本法适宜作为大规模血清流行病学调查的工具。

Abstract: A modified ELCOPT technique using heat-treated, ultrasonicated dry egg antigen was reported. The relative sensitivity of this method in 327 parasitologically confirmed cases was compared with those of COPT, ELISA and IHA. The positive rate of ELCOPT was 96.0% (314/327) as against 94.2% (308/327) of COPT, the difference being statistically insignificant (P0.05); the average percentage of ova with precipitates in the former was 27.9% as against 15.7% in the latter, the difference being highly significant (P0.01); the positive rates of ELISA and IHA were 89.6% (293/327) (P0.01)and 82.1% (87/106) (P0.05) respectively. Sera from 43 healthy persons all showed negative with ELCOPT; when sera of 87 children tentatively diagnosed as hepatitis below 15 years of age were assayed, only 3 cases (3.5%) produced positively colored precipitates, the percentage of ova with precipitates was 1-2%. The microplate method of ELCOPT presented here using heat-treated, ultrasonicated, dry egg antigen and the appropriate substrate, 0.4% benzidine (4,4-diamino-biphenyl benzidine) might be a useful tool applicable in large scale seroepidemiological surveys.
